Materials Required:

1. Essay: Scholarship candidates must demonstrate their interest by submitting a type-written essay. The essay should be no longer than 1,000 words. The essay should use the CHIME Seven CIO Success factors as the basis of a thesis to address the candidate’s vision and goals as a future Healthcare information technology leader.
2. Letter of recommendation: Candidates must submit a letter or recommendation from their immediate supervisor. The letter should support the applicant’s attendance at CHIME’s educational events and explain why their participation would be beneficial to the candidate’s professional development. The letter should also support the fact that applicant is on a CIO career path and that attendance at CHIME educational programs is important to their professional development.
3. Resume: Scholarship candidates should provide a current resume highlighting their experience and/or achievements in the healthcare IT field.
